For the application of the agreed fares the patient shall previously inform his Insurance Company and supply the following documents at the check-in:

ID document
Tax number
Insurance badge or policy numbers
Medical Certificate stating diagnosis as well as past and recent anamnesis and requiring the hospitalisation.

This Private Hospital will forward the request of authorization for hospitalisation to the patient’s Insurance Company directly; he will be asked for support if needed, in order that the cover authorization from the Insurance Company has arrived when checking out.

If the Insurance Company covers the expenses, the patient shall only pay the extra due amounts and possible franchise, signing a commitment letter when checking out. CASAGIT agreement: in this case the patient shall also pay the foreseen local tax and supply the Casagit form.

In case the Doctor working at “Pio XI” has no agreement with your Insurance Company, the patient shall previously advance money to pay the medical expenses and then address to his Insurance Company for refund.

If no Insurance Company cover has been authorized, the patient shall pay the full amount (private hospital expenses + medical expenses) and then contact his Company for reimbursement. FAS agreement: the FAS form is required.
